I find the best thing you can do with USB connectors is make a block by taping them together with electrical tape. As for labelling:

+5V = Power
+D = P+
-D = P-
Ground = GND

Just make sure the (1) and (2) connectors are all on the same line in the right order. Plug Shield into dummy.

Your power and reset connectors want to go over their respective pins and a ground pin, so reset switch goes onto RESET and GND.

The LEDs need the right polarity. On the side of the HDD and Power LED connectors that don't have the writing on, you will see a little black arrow. That signifies the positive pin - match that up to the respective positive terminal on the motherboard.
