So after a few years out of action I come back to realise no one plays COD MW2 multiplayer anymore?!
So I need something similar to replace it, mindless shoot-em-up action fun with minimal thinking required
Thank you!

Single or multiplayer ?
Society's to blame,
Or possibly Atari.
Nothing is more mindless than quakelive.

oh it's free. perfect
anything else a little more COD-like ? I will download and try MW2 because apparently now there are 3k players on there, when I tried before there were about 300 and I couldn't get matched at all
Suvarium is a fairly new title and this is a free title. I would suggest looking at some of the free to play games first as that lets you try out a few of the different game engines which will enable you to see what sort of thing you might want to play moving forward. Here are some examples:
Battlefield Free to play
Warface was built on the Crytek engine.
Blacklight Retribution which I think was built with the Unreal Engine.
Gotham City Imposter and Team Fortress 2 might also be worth shortlisting.

There is always TF2 which I see some people have suggested which I also do recommend!
However, I would suggest you have a look at a game called Blacklight: Retribution which follows a pure Sci-Fi Multiplayer FPS. It does have some sort of competitive aspect to it, but so does all other FPS's including TF2. Nevertheless, it is pretty enjoyable to play the different game modes such as Onslaught or Siege, including the all loved TDM. There's is a brief outline on the different game modes and features (such as the HRV) on the game by GiantBomb (I would link it specifically but alas, I currently do not have enough posts).
The game also has a vast weapon and armour customisation system which makes it all the more fun. Also, did I mention you can ride / deploy an insane machine gun implemented hardsuit (a mech)? <- Believe me this is crazy!
And if you haven't realized by now, this is also F2P!
EDIT: whoops, I've just realized this game has already been mentioned
Cube 2 - Sauerbraten (free ; not f2p BS)

Just got given a copy of some co-op FPS involving gunning down dinosaurs. Orion: Prelude I think. Couple of quid on Steam.
Jetpacks, lasers, tanks, mech suits and all kinds of stuff, while you face off against raptors, T-Rexes, Triceratopseseseses, stegosaurs and other such...
Quake becomes pretty mindless if you have decent aim, eventually it just all becomes instinctual, it's quite relaxing once you reach a good level of aim and game sense. And by decent you may realise your aim actually sucks when you start to play quake.. took me years to get used to it.
Other mindless FPS games without a skill ceiling? hm Team Fortress 2, I think it is the third most active game in steam (Dota 2 and CS:GO being the top 2).
If you aren't "competitive" and just in it for the fun why not look at the Co-Op games? (Left 4 Dead 2, Killing floor)
Not quite first person: Arma 3
