Re: ECU Remaps
When I had my Golf PD 150 remapped by Revo I had no end of people telling me it'd need a new clutch. I had the remap done when the car had about 10k on the clock and I sold it with 96k on the clock and the clutch was still perfect.
Next up was the R32 (which I've still got). I had a stage 1 Revo map put on and was very happy with it. Revo have a reputation for giving a digital throttle but the throttle response is adjustable between 1-10. I had mine set to 6 and was really pleased with it. Being an NA car it didn't get anything like the gains that the PD150 did, but it smoothed out the power curve which made it easier and more enjoyable to drive rather than just all out faster.
I had the Revo map on for about 8 months and would still have it but I went for some engine mods that meant a custom remap was the best option.

Re: ECU Remaps
Yup, insurance doesnt actually bump that much. Although, like you, many people say dont bother.
Anecdotally on a BMW forum or two, most are quite happy with the work done though typically the company varies.
The company I'm looking at does have good reviews as best I can locate, though they're usually not specifically BMW.

Re: ECU Remaps
Insurance may, or may not go up. Some insurers will flatly refuse to cover a remapped car, others will do it but increase the premium and others it'll make no difference.

Re: ECU Remaps
I did notice oddities getting quoted for it, like they tended not to ask the kinds of question that I would have and given the small hike for the substantial performance difference suggests its either a catchall increase for the company, or they've no idea and are just slapping a trivial non-specific "modified"/"non-standard" premium on top.
/shrug. Their problem I guess, another 40 quid a year for a 48-50bhp hike seems cheap :D
