For me, the best scores has to be the ones that best keeps me going. Also happens to be some of the better scores, in general. While I really, really liked the OST for Red Dead Redemption, my favourite is the one for Silent Hill 2. Akira Yamaoka really did something amazing there.
The one soundtrack that can take me through my full spectrum of emotions and remember every little teeny part of the game, and the periods where I've been playing that.

Xcom 2. It sounds like a bombastic action movie in all the right places. The squad selection screen for example, and at the end of the mission when they do the slow walk to camera.

World of Warcraft OST. Not many know it exists but once you disable all other unnecessary sounds the game the game actually becomes way better.

Civ 4 - the Baba Yetu theme track deserved its plaudits but the rest of the soundtrack was excellent too - for a game that sucks you in for hours/days/nights/years at a time, it's rare to have a soundtrack that doesn't grate within a couple of weeks.
Machinarium - the soundtrack (by Tomáš Dvořák) is a good fit for the game's aesthetics, and also brilliant in its own right.

Flatout 2 - a lot of awesome rock/metal, intruduced me to it when I was younger
Fallout 2 - it was just a perfect atmospheric music. First time played it through without touching the radio, second time It was on all the time.
final fantasy x - whatever they were playing in those "churches" or what did you call them? So long ago

For me, the soundtrack from Dark Souls, Undertale, Final Fantasy XIII and Halo 5 are the best.

Re: Best soundtracks you've heard in games?

So many great soundtracks, so many to choose from.
    Not in any particular order.

Fallout - The series always has had good music, but the first/second one really captivates me more than the (also great) radio musics from F3 and NV. My two favorite tracks by far are Radiation Storm used in the Glow, an abandoned research facility abandoned when it was nuked 80 years ago, and Followers of the Apocalypse, most memorably used for the Enclave, the remnants of the US government, in Fallout 2. Gives me chills every time.

C&C - Red Alert 2. Frank Kelpacki is a musical genius, and the action packed RA2 OST was perfectly fit for every second. By far the best track of the game is Grinder

C&C - Tiberian Sun. Much bleaker and dark music than RA2. Likewise fits the themes of the game perfectly. Favorite tracks are Pharotek and Heroism.

Deus Ex & Deus Ex Human Revolution - Really awesome series ('cept Invisible War) and really awesome music. If one has any taste for a good story Deus Ex are definitely worth a whirl. Chosen tracks from Deus Ex 1 is the Main Theme and from Human Revolution is Icarus. Icarus is probably the single best game music piece I've ever heard.

All tracks worth a listen, even if you've never heard of the games.

Agreed the first C&C and Red Alert 1 were great.
