Weren't there some Elvira games for the Amiga which were a bit ...erm... titillating? Not that I ever played them. Oh no, not me.
The best "adult" game I've played...
There have been all sorts of games over the years that have featured midly titillating imagery but for my money even to this day Silent Hill 2 manages to be one of the best video games out there with real adult themes that isn't just trying to steer teenage boys towards a sales desk by showing some skin on the box art. It all seemed to fly completely below the radar as well as far as generating controversy went... Here was a game with themes like suicide, sexual frustration and child abuse that handled them seriously and all the censors and the critics seemed to pick up on as "adult content" was the gore.
More recently I guess I'd throw Until Dawns hat into the ring. Yes it's campy and lives off of horror movie tropes that we all know but it manages to be self aware and provide an experience tailored to an adult fan of the horror genre that's engaging, enjoyable and one I intend to play through again. I know they're completely different interpretations of an "adult" game but it was good enough to get me stop playing MGS V for quite a while...
the best game that countd as purely wrong and heavily needed to be adult only?

a toss up between Leisure Suit Larry (6 i think?) or Carmageddon 2 depending on your definition.
How about Dracula on the C64 by CRL, text adventure with a few gory animations and scenes, it was actually awarded a BBFC 15 cert, a new thing for games back then.
Techno Cop by Gremlin on the C64 and Amiga created a stir when released, you could either net bad guys with your gun or just shoot them, when shooting them with the normal gun they exploded in a shower of red goo and left a carcass on the floor. Thing is if a civiliian happened to get caught in the crossfire then they would do the same thing. Not a bad game but the loading on the C64 from tape was horrendous. I still have my copy from the 80's just to my right at the minute but don't play it due to loading.
Like previously mentioned there is the Leisure Suit Larry series, I could only ever play the cut version though as I couldn't answer the questions at the start of the game on the Amiga, I had a dodgy parrot on the shoulder version but they didn't remove the age check!!
Ahh yes, I remember stumbling across Leisure Suit Larry on our family computer yonks ago and trying (never with any luck) to answer the questions... the age check definitely worked! Pretty sure I still wouldn't be able to get into it now though...
As for 18+ games I have to be predictable and say I have always found the GTA series enjoyable, particularly GTA V.
