On a brief glance it looks like her neck is part of a mask covering her head, but if you look closely, you'll see that her black jacket over her black dress creates the appearance of loose squared skin. I personally don't think it's a photoshop.

Yup. Black jacket over black top/dress makes it hard to see but it appears her top/dress has some sort of strap or clasp across it. Definitely looks like an artificially attached head, though.
