I've got a server running 2003 'testserver' that runs IIS. I need to allow this server to be externally visible, but having never touched ISA I have no idea where to start.

Which version of ISA? I assume that DNS is setup, and that the dite you're talking about is running on port 80?
You need to run the publishing wizard and select "Publish a Web Server", the wizard will fill in all the defaults for you i.e. port 80 etc. you will just need to enter the internal IP address of your web server.
Check out www.isaserver.org for loads of excellent (free) tutorials, if you're looking to do more with ISA you could do a lot worse than buying Tom Shinder's book (advertised on the isasserver.org website) also.
