I do not have direct control over our websites DNS records and any changes need to be sent to our hosting company to be made.
I requested a new subdomain to be added to our domain name eg sub.mydomain.com and pointed to our IP address. I did this via email and also a faxed and signed document
The email and fax went along these lines... (I am using a fake IP)
Please add a new subdomain to our DNS record
Sub IN A 10.10.1.1

So what did they do on Saturday night??
Change our MX record to point to 10.10.1.1 so all email was bounced back to sender.
How can anyone make this mistake??

all about their specialisation. I have only dealt with 2 companies directly however, one in the UK and one in Dubai. I'm sure that most are actually pretty good or else the internet as we know it probably wouldn't exist.
