Well, maybe in a few months (once I've rebuilt the HTPC) I'll have another look into it. I'm anticipating something along the lines of either a single 120 rad above the motherboard, or a couple of 80s one on each side of a 9cm - 10cm deep case. Hopefully around Christmas some new motherboard will come out (I'd quite like to do something with a 780G / 785G and Hybrid crossfire ) and I can start considering it seriously...
The Corsair H50 looks interesting:

Interesting thought - might fit quite nicely into the SG05 or similar (though I image the drive bays would get in the way of the front 120mm mounting point and that it would fall foul of the optical bay even if you mounted it on the front - i.e. outside - of the case)
I suppose if you are using a the Zotac 9300 mini ITX motherboard or one of the 780G or 8200/8300 based ones you could use the side of the case to mount the radiator couldn't you?? Of course this means no use of the PCI or PCI-E slot some of these motherboards have and you would have to mod the case though.
Also you could replace the stock PSU with the PicoPsu 150w and a quieter 120mm intake fan.The radiator could then be mounted at the back in this case. I would imagine that one of the mini-ITX motherboards with an Intel E5000 series or or lower wattage AMD Athlon X2 or Sempron,a slimline optical hard drive and one hard disk would not need more than 150w even including the water cooling system and fans.
However, I could be wrong though!

I had a whole variety of thoughts on this. One way I can see doing it would be to get a case that doesn't take an optical drive (something like this one), and mount a 120mm rad *outside* the case above the motherboard with a 120mm fan inside the case blowing out through a modded hole. The other would be to go with this cube case which (based on user reviews I've found) would take 2 single 80mm rads *and* a graphics card (780G + 3450 in hybrid crossfire, anyone ).
Today, I've been drooling over some Pico-ITX kit though... case, slim CD (with adapter), Atom Z-series board, DDR2 RAM, 32GB Compact Flash... what a lovely, lower power portable system...
Have to say this is my favourite pico case:

Also - if you're looking at that lot have you considered the Fit PC 2?

(I really must ask if I can get a referral account from them - that's the 3rd or 4th time I've been pimping it on these forums!)
Oh, I'm not really considering it, just drooling over it I like the Travla case because it supports an optical drive, which most pico-ITX don't. Gives you the complete all-in-one package.
Fit-PC looks a nice idea but has some odd implementation ideas - no VGA, for instance. Good price for the kit though, considering it's the same innards as the pico-ITX board I linked - I might consider getting one and modding it into something portable
Fit-PC...wow. Need more info as that looks splendid, might have to get one...
