One of the worse has to be Dark Souls - Such a terrible port that required community patches to even become playable.
I was playing Final Fantasy XIII the other day and it seems to of ported over quite well (looks good, has some some basic options) but has its issues like a MASSIVE 50+ GB install and you can play the game 100% with the controller but can't exit the game without a keyboard and mouse!
Max Payne 3 PC had a lot of love from Rockstar but deserved it since it started on the PC.
Then we have the games in my mind that were ruined due to be consolfied - Deus Ex 2 which the main one that jumps to mind.

GTA IV was unpleasant. The combination of it playing terribly and not particularly engaging made me give up on it pretty quickly. Sounds like they're going to do V properly, at least.

GTA 4 was a bad port, Resident Evil 4 was the worst though, didn't even have mouse support, lol
One of the worst console ports has to be Batman: Arkham Knight so bad they had to stop selling it.
One of the worst has to be Dark Souls even though it's my most played game on steam. In the same category I'd include LA Noire and GTA4 (though I liked them as well).
As for the best, dunno, but I was surprised at how good was the GTA5 port.
Best console port has to be GTA 5. OK maybe not really a port but good to see that they spent time making it better for the PC.
I have some sort of Sonic racing game on origin (or some other non-Steam DRM...) that I can't even get past the main menu in. The buttons are truly all over the place! GTA V was done well but it's a tad annoying we had to wait as long as we did
One of the worst for me was Assassin's creed 4 black flag(I haven't played any of the others). In game Vsync didn't work properly(FPS drops), when VSync was disabled it had some of the worst tearing I've ever seen in a game. Nvidia control panel vsync corrected these problems, though. Also, their SMAA implementation was very bad and didn't remove many jaggies and seemed to introduce a strange sharpening effect. The FPS was very inconsistent and poor in general. If you enabled physx it destroyed the framerate no matter the system. I just think Ubisoft do a poor job in general with their PC ports.
Metro last light, last light redux and 2033 redux are all very good, GTA V is reasonably good barring the stutter issues introduced in the recent patches, Witcher 3 is good after patch 1.08 and Grid 2 has always run very well on my system as does Dirt Rally(not a port) and Crysis 3 was good after drivers fixed some initial issues (took a while), although some of the physics may have been more efficiently done on the GPU rather than the CPU or a lower level API was needed.
I get that a lot of people don't like the ports of Dark Souls and Witcher 3, but I use an Xbox controller anyway for all games where the camera is centred on your player. I found I prefer the combat that way. That said, Dark Souls was a poor port with the graphics problem where you needed DSfix.
