It's a toss-up between my Quickshot 2000 joystick for Sensi Soccer, that stupid dongle thing you had to put in to play Robocop, or my MX518, which has lasted me about 9 years
Edit: it wasn't a Quickshot,it was a Competition Pro 2000. Like this:

Microsoft sidewinder forcefeedback2 joystick. Still use it today. Occasionally I have to replace the odd faulty button micro switch and it still works like a dream.
My kempston quickshot 2 joystick with the auto fire button. I still have a couple in the loft with my old Sinclair Spectrum 128 (the original one with the radiator heatsink on the side)
Smudger, that Robocop 3 dongle isn't actually needed, there is a printed number block at the bottom of each page in the manual which is an alternative and the game will give you a few grid references to get past the copy protection, they removed the dongle completely on the Atari ST version as the dongles weren't overly reliable and prone to failing. I use the dongle on my Amiga 1200 but my 600 you can't actually plug it in with the ports being on the right hand side.
My favourite has always been the Powerplay Cruiser, I personally prefer the blue one as the ball on the stick is the same as the pictured zip stick ands the other colors have a tapered off ball, the base is thinner which I always found was easier to hold.
My blue cruiser, I need to replace the switches in it, currently they have slide on connectors but I need to cut the connectors on the new switches as they are to wide, so just going to solder wiring on, not exactly rocket science and cheaper than a dremel!
Another vote for the competition pro. I got one with my first Amiga (still in use, but the silver has worn off to the green plastic underneath on the knob)

I've got a JogCon somewhere as well - that was cool for half an hour
I'm also stockpiling the PsOne official screens - hard to get decent ones at decent prices and the picture on them is fantastic.
Mine has to be a suite of related peripherals...
The Fanatec CSW v2 Wheel Base, Club Sport Pedals, CSL Seat and CSL Xbox Wheel Rim
Most of it is the same as what is in this picture.
I am planning a little PC upgrade early next year to allow me to add 4 tactile feed back units at each corner for added realism...
Contemplating a Vive as well
NAMCO Guncon 2!
A chair.
A mouse (serial or ps2), most people can't imagine a PC without one (no touch / pen input).
Just a screen, a box and a keyboard. and a blinking cursor. . . . . .

I was cursed with only being born with 2 hands, so for me, the Makopad 64 was the best peripheral;
The worst was this joystick for the Atari ST - absolutely useless for jumping between platforms;
CH Products Hotas (FighterStick, Pro Throttle, Pro Pedals)... As indestructible and accurate now as when I bought them 6 or 7 years ago.
I Loved the Namco Negcon, the control it gave in support games like wipeout was epic and the laugh as other tried and failed to use it was great.
Also my namco arcade stick, got me through many games on the ps1/2.
In more recent times it would have to be my Rift.
