What filter drivers do you have installed?
(Anti-virus, virtual device drivers, firewall, backup agents, etc.)
NTFS.sys may appear to be the driver owning the I/O request, but a filter driver most likely screwed around with it.

i have had quite a few vista crashes on my htpc - all sleep function related and wake issues
some issues with divx codecs too with COM stopping working
rather annoying overall at present
not had a blue screen from this, but had a world of hurt with badly installed virus software, cuasing hardware lookups, this was rc stage thou. Seams the most common culpruts.
(also had a loverly issue with windows media player crashing, due too flac codec)
