Music streaming apps

Oddly, I am finding it difficult to find a piece of software!

Wondering if anyone knows of any apps that can present your music collection in a web interface for streaming over the internet?

It can be Windows, Linux, FreeBSD or a virtual appliance. Must support reading of music from a CIFS/Samba share though.

Lots of web frontends and can encode for streaming. Best part is it's small a light and will work on almost any hardware.
