Permanently confused
I have a Microsoft Laser Wireless 5000 which has the extra button on each side. So left side button goes back over a page when browsing the net. The right shoulder button is the magnifier which I have found I have had to start using lately with the higher resolution on my monitor (actually had to increase the browser to 120% also - must be getting old! ). If I hold the right mouse button down I get a disc up which has forward and back etc, use it for browsing again.
So literally it is a case of index finger and middle finger on the top of my mouse.
Browsing/using GUI - Index and middle fingers on top.
Gaming - 3 fingures on top.
Either way, I normally have my wrist on the worktop for more controlled movement.

Using two fingers on the top, as opposed to three, means you can get a firmer grip on the mouse. Not sure if that's relevant because if you've been brought up with three fingers your hand will naturally be stronger anyway. Most people don't have particularly strong pinkies unless they're musicians.
I tend to use a mixture. I actually use middle finger to scroll, but my 'idle' position is index and middle on the two buttons. Either I need to use left and right or left and scroll, rarely all three.

Hmm ya know, this is actually a pretty good question.
I just use two fingers on the top of the mouse, guess I'm just used to it that way...
