GeForce GT 520, hoping to buy a R9 285 or GTX 6/770 or something soon, or 970 maybe. I'm not sure.
HD6870 @995/1150 MHz playing Titan Quest. It looks awesome with the right filters. 8x SSAA, SMAA & EQAA(by proxy of RadeonPro) and +0.6 LOD(newer gcn ones don't require this last step).

Sapphire Radeon R9 270X. If I had the money, I'd upgrade to a 290X, but they are a tad expensive in Australia ($600+).
Got a GTX670. Still powerful enough for my needs, but I'm eyeing that GTX970 enviously...
MSI 560Ti 1GB Twin Frozr. Could do wih an upgrade, will wait to see how the 960 fairs...
intel HD400 . But I plan to get a 980 GTX G1 from gigabyte next month. Had a 780 gtx that died, from Palit -.-.
Another Sapphire 270X (Dual-X)
It's alright for me, if I had more time (and cash) I'd definitely upgrade. But it plays everything I've tried so far rather well.
750Ti does everything that I need.
ASUS GTX 670 4GB - More than enough for playing World of Warcraft at 1080 resolution on a 27 inch screen. Even the old GTX260 which I put into my wifes PC manages WoW easily enough on the same size screen.
No plans (or money) to go 4k yet and really see no reason to as it doesn't have a great deal of support in games. I'd rather go multi screen than one ridiculously expensive screen needing a £800 graphics card to run it at any reasonable frame rate!
I am using the HD6950 1gb graphics card along with a newly upgraded system with ROG mobo and core i7. This is giving me excellent frame rates to play the latest games at mostly High settings at 1080p resolution. I am quite satisfied and really don't see the need for an upgrade to the graphics card yet.
EVGA 670 GTX SC in SLI
7870 XT (Tahiti LE) 2gb. It's coped with everything I've thrown at it so far, and as I've gone back to playing WoW I expect it'll last a while longer yet! I'll pick up Dragon Age Inquisition when it's on sale one day, that will probably be the first game that'll tax it.
4GB ASUS GTX 970 STRIX
Upgraded from a 1.5GB ASUS GTX 580 DirectCU II which was starting to show its age and couldn't be happier.
Plan to pair it up with a second in SLI when I can get my hands on a 4K GSync/FreeSync (assuming NVIDIA support) monitor for the right price.
Palit GTX 470 soon to be replaced by an EVGA FTW GTX 970 (was temporarily an EVGA ACX GTX 770, the 470's a backup card, EVGA step-up to 970). Had it for about 5 years now. Runs hot as hell, loud as it can really be, but still performs pretty well and has surprisingly high OC potential (not that I use it for fear of going deaf), currently sitting at 720mhz at .962v (lower than stock volts with nearly 120mhz OC). Can still push 120fps on basically every game if I turn the settings down.
And another machine with an ASUS 760 DCUII Mini which doesn't really OC at all without becoming rather loud.
Lastly a laptop running a GT630M, the slow, DDR3 equipped one. It's kinda there. Plays stuff. Useful backup, at least.
