Quickshot Maverick joystick for the old Amiga. I went through a few of these (especially with StreetFighter 2) but loved them all so much
Probably my first Amiga 600 mouse followed by the XBox 360 control pad.
I nominate... my hands! Specifically the right and the left one. Couldn't play games without them.
I find it hard to think of anything that is more useful than a mouse...
Especially after they developed lasers to replace the ball... and decent wireless mice these days don't suffer from delay.

got it all bought it 10 years ago... it works like when it was new still... best quality ever and something companies liek Razer could learn something from.
I know it's a boring choice but probably the humble Xbox 360 controller.
Dualshock 4 for console gaming, I have smaller hands so the DS series fit them well. The DS4 packs loads in, works well with the PC and some how manages to have a touch pad in.
Cherry MX Browns on a mech keyboard (currently Gigabyte) + James Donkey Avago Optical and Omron mouse, 3m pads and Func 1030.
Seriously the Func 1030 is my favourite mouse pad of all time, I've got my original Alienware (pre-Dell) pads. They are amazing. I do a lot of Photoshop work, so I like very accurate very low friction mousing. I colleague called it an ice skating rink.
Amiga Era I liked my Zip Sticks.
Current - 360 pad
Previous - Powerpad Pro (all about that throttle slider!)

Very difficult question but a good one.
I would have to say it was "the expert cartridge" for Commodore 64 back in the day(Yes I am that old).
Basically it allowed you to freeze program in memory so you could search for and add POKE's to cheat in games, Give you infinite lives and such. That is all I used it for although it had other purposes for copying and such.
Man, my good old Qhickshot/Quickfire series of joysticks, for use with my C64.
Broke so many of them waggling them from side to side playing the likes of Track and Field etc.
Damn, good old days when having a twin tape deck stereo was the best thing to have... and not for playing music on

I had one for the Atari ST. Kept breaking, ended up covered in screws and bolts to hold it together and keep the switches operating cleanly so it looked like something from the Borg in the end, but my word that thing got some use. Specially playing Gauntlet II
