Would a powerline adaptor work?
You could look at whether your router has a removable aerial. If so, you may be able to replace it with a higher gain one / a directional one.
There's also the option of a directional antenna on the receiver end.
Powerline no as outhouse has separate supply from house. Router doesnt have a removeable arial. Would a wireless extender be good?
My parents used one with mostly no issues. That said, they just browse. If you're gaming or even streaming a lot it might be a different kettle of proverbial fish. I suppose it depends also on how much money you're wanting to throw at this. I've heard of someone else using a powerline repeater (so basically repeats wifi at the end of a powerline signal - kinda the same as what you're suggesting) and this worked well enough for kids who play games so fundamentally I'd say the repeater stuff may well be just fine. My concern would be latency.
Provided the outhouse is on the same metered supply (and phase - in the unlikely case that you are on a 3 phase supply) you will probably find that power line adapters will work. Simplest and cheapest method would be to run a length of cat 5 or cat6 cable.
However I see you have ruled that out - presumably because it crosses a road or path?

Yes, correct, a road.
Powerline should work if the power comes off the same circuit breaker box. That's probably no worse than I have in my house where garage, upstairs and downstairs are on different ring circuits. From ring to ring I get about 40Mb/s using 500Mbit powerline adapters.
