XFX 6850 Faulty

I wrote to XFX support and they have said to contact Scan regarding the faulty graphics card. Tried it on various systems and OS and it produces similar phenomenon - lines/blocks of colour appearing all over the display rendering desktop unusable.

Whats the procedure for returning this to Scan to get a replacement or different product?

EDIT: I will place an OLQ RMA request when I get home but how much will it cost me to get it back to Scan?

I am at a point where I am utterly frustrated with the situation on the card. Scan ask me for an XFX Reference number, XFX don't provide and pass the buck back to Scan saying that a serial should suffice and in the mean time I am having to call during work and by the time I get back from work with any email updates, the lines are closed. So this continues until the next day.
