does your hosting have cpanel or similar software? if so then there is the easiest way :]

I thought I saw somewhere you can do it with a PHP script but I can't think for the life of me how that would work!
Well firstly the name has to be added to the DNS for the domain, then the server which hosts the domain must then be configured to accept name-based/ip-based virtual servers.
For example with apache: http://httpd.apache.org/docs-2.0/vhosts/
Though as previously said there may be a control panel that will do all this for you.

you need an A record in DNS or a CNAME alias pointing at another domain then a host added on your webserver to use this domain.
These aren't actually called sub-domains so don't ask you ISP for sub domains as this isn't what you get
forums.hexus.net is not a subdomain - its just a host record in the hexus.net domain. no different than www.hexus.net is just a host record.
a subdomain would be for example.
forums.prod.hexus.net
forums.dev.hexus.net
You just want another hostname adding.
