I got mine, pretty much bang on a month after sending the paperwork away. There was an email address at the bottom of the sheet you fill in in case you needed to query it.
Do you still have the .pdf? Should still be on Xbox.com if not I guess.
Cheers but wasnt the xbox add on one but a toshiba one , have deleted the PDF file its been 2 months now need some compensation for betting on the losing horse !
Well my dvds finally turned up 2 months late and totally different from the 5 i ordered
So glad i brought a hd-dvd player
