I meant the pain he was in with that.
There's a surprising amount of "empty space" inside the human body, what generally happens in these kind of scenarios is the organs are pushed aside rather than actually perforated. The biggest risk isn't from having your heart or lungs pierced (the rib cage tends to deflect the blows), but of having a perforated bowel which leads to a slow, painful death.
