1. Whether you'd completed a risk assessment for the aforementioned act
2. Had your ladder (anything with more than two steps constituting said item) safety tested
3. That you write a standard operating procedure for changing the lightbulb
4. Ensure you have a glass breakage procedure in place
5. The area is cordoned off whilst work is progress
Having waited until after you'd changed the bulb would mention that you should have got the work done by a trained electrician and then gives you a safety non-conformance to investigate and fill in, complete with an action plan to ensure that the "Saftey Incident" never happens again
If it ain't broke, fetch a bigger hammer

um, what sort of light are we talking about? screw fit, bayonet? halogens gr11/16? gu? HIDs? LEDs? strip lights, neon lights, flourides...? i could go on but then we'd never get it changed
