Ok - just tested during a session on COD4, and GPU temps went up to 58-59 from 46C.

Can you try pressing down on the graphics card to make sure it's in the slot as far as it goes? Also check that there aren't any cables or wires touching it inside. Also check that the lead to your monitor from the card, is plugged in tightly, and that it's not twisted badly or anything.
I also would do a complete uninstall of your graphics driver, ideally with a "driver cleaner", which gets rid of all trace of it. Then reinstall a different one. The latest one from nVidia.com, and if you where already using the latest one, use the second to latest, or a beta driver. Just as long as it's different, it could help rule out the driver being corrupt/a problem.
Also, does your monitor have a driver? If so, you could try using that. Usually the screen will just be a "plug and play display" unless you install a specific driver. That could maybe make a difference.
Make sure there are no speakers or TV's or stereos or routers or anything close to the screen or the cable to the screen. Just to help identify.
If all that fails to resolve it, you could consider uninstalling your graphics card again, and then setting up the graphics chip on your motherboard - assuming your motherboard has one. Most modern motherboards do. It will be a bit crap, so it won't play games well, but if you could play a game with low settings and see that the lines have gone, then it will show that it's the graphics card (or something related to the graphics card - like the PCI port or the lead to the monitor from the card etc).
No probs. Hope one of them works. If not there are other things to try.
