Track IR is very good once used to it....CLingy uses it.
And Pedals too. If you could map game movements to pedals it would be ace for him

Actually wasn't there a mouse that had loads of little joysticks over it? Could use one of them for movement.
An even better solution might be the Actuator from OCZ.
We first saw this at CeBIT 2007 and I've just got back from having a play with it over here at Computex 2007 in Taipei.
Basically it senses electrical impulses in the face, which you then assign to various bands in the software, and then assign that band to a key press.
So you might lightly clench your jaw to run forward, then clench tighter to run backward. Twisting your head very slightly to the left or right gives left or right straffing... and voila... you can now circle straffe and move.
It was dead easy to use and much less troublesome than I thought it'd be.
Apparently, with fine tuning, you can get six different actions into it with practically no visible facial movement.
