Only once. Went to Barcelona recently, only used the card in one place, a restaurant, and they cloned it.
Fortunately the bank spotted a couple of odd purchases and let me know. Didn't loose anything thankfully.

Had a couple of transactions on my card from PayPal, got them sorted out and changed all my Paypal details. A couple of months later there was a few bad transactions directly on my Mastercard. They were sorted out really quickly and efficiently by Partnership Card Services (John Lewis).
Yep. No idea how though as the card that got done over was very very rarely used. It was used in Sweden, Belgium and Holland (places I regularly go, but I'd never used that card there) to buy 2 train tickets and a flight from Brussels to Madrid (I think).
It was a pain to get sorted, the bank decided that as I hadn't filled in and returned the forms they sent me within a week I must have remembered that I did actually make those transactions so they put them onto my account and wanted an interest payment. This is despite me notifying them of the fraud while I was at Heathrow waiting for a plane to San Francisco and telling them that I'd be out of the country for 3 weeks but if they wanted they could fax the forms over to the missus' place and I'd sort it there.
The company card hasn't had any problems. Considering just how much use that card gets all over Europe it's a surprise it hasn't been done over by now.
About 5 years - Natwest Switch - £150 was paid to T-Mobile. Bank paid out quick enough. Dont know how/where they got the details from. Wasnt doing too much online at that time.
Barclaycard visa - Attempted purchase of £2400 worth of flight tickets and furniture on my student barclaycard with a credit limit of £450. Obviously they didnt get anywhere, and barclaycard replaced the card.

Having thought about it, there was some very iffy fraud back when I used Natwest. On this particular account I only had a cash card - no switch card.
Imagine my surprise when I saw Switch card withdrawals on my statement! I went into the branch and was told that I was mistaken - it was impossible for what I was saying to have happened. It wasn't until I showed them the cash card that they believed me.
After about 4 months the money (only about £20, but that isn't the point) finally got back into my account.
How is it possible to buy a flight on a stolen credit card? Surely the thief would have to be the most stupid person ever? Name, passport details etc would all be logged?
Who knows. Thats what barclaycard told me they attempted to buy.
It wont be a problem if they never intended to return to this country. Only as bad as taking a £10k higher education loan and shooting off to Vietnam or something like that - someone my friend knew did that.

Man, it's scary how many people have been frauded on this board alone, and you can add me to the list.
I had my switch card frauded about a year ago. Tried to draw some cash from an ATM but it wouldn't let me. I thought that was wierd 'cause I just got paid a few days ago so I checked my online statement.
Guys bought a couple of plane tickets with my cash, and went over my overdraft too. Luckily HSBC managed to sort it all out and I got the money back, though I was about two weeks with no money to spend at all.
Now I only ever use my credit card rather than my debit.
Not so far (and hopefully never!). What I despise though, is finding out that a direct debit that should have been cancelled but has not. Darn Allianz! Now pick up the phone so that I can inform you to forget about leeching £5.99 off my account this month!
Edit: Well, least it seems sorted in 9.5 min (including 6.5 min of waiting) over the phone. Now have to wait 10 days for the refund.
