Could me a load of different reasons for this.

Mostly likely a sound card or a game / program that can affect the sound settings at a computer level. Uninstall sound drivers?

I even have a keyboard where the volume button is now too heavy for the spring beneath it and so my volume decreases on it's own. Pain in the butt.

If I ever get a niggly problem like this that lasts after resetting the BIOS and turning off hardware [or removing it], then it's time for a system wipe and a fresh copy of your OS.

I've had a similar problem for ages with Vista, is it by any chance, at 65% at boot?

Not got around to fixing it, as I rarely reboot, it doesn't cause me much issue. My best guess has always been it was something to do with headphone detection. As I've noticed the same thing happens if I switch between my hifi and headset while the computer's on.
