I've used Silvo (silver polish) wire wool in a tub quite successfully on PS2 games and a badly scratched DVD... it's a bit rough but gets rid of the rougher scrates, and leaves lots of littler scratches...

My uncle used to swear by toothpaste, rubbed into the scratches - whether this works or not I'm not sure

Also, if you are having problems copying the data off just using Windows, use a CD/DVD ripper app. like DVDDecryptor (for example) they often have much lower level reading methods for use with scratched disks.
The actual drive has a lot to do with it too. If the data warrants it, get a hold of a Plextor Premium drive (if you can) they are really, really good at reading damaged disks. Their audio ripping accuracy is outstanding too.
I know my local Choices (video rental chain, pretty much Blockbuster but not) used to offer a service where you give them the game, they send it off to get repaired. It wasnt too bad price wise, altough never used it, so cant comment on how effective it actually was.
Sadly I believe Choices is no more, well at least my local has gone. I believe Game offer a similar service?

Gamestation and Chips do a repair cd/dvd service. I think a lot of the independants now do it.
Toothpaste does actually work pretty well, its only midly abrasive so could take a few goes before you remove a thicker scratch. Just make sure you use a soft cloth when applying and you may need a little water as it dries out quite quickly. Also remember to do it in a circular motion.
You could invest in a CD cleaner, or if it is only one disk you can buy cleaning solution and cloth. I've had mixed results with different machines.
If you do a search for "cd repairer" on maplin.co.uk then there are various cleaning machines.
There are a load of home remedies that are talked about but very rarly work. I'd go to somewhere like gamestation I've taken a couple of really badly scratched discs in and they have always fixed them for around £3. Can't ask more than that.
