Corsair M65. I love it.
MX518 here too, also have a MX510 as a back-up.
Newer mice are not the same quality, my youngest lad had its supposed replacement model - the G400 that lasted just under 12 months, was replaced under warranty by Logitech with a G400S.
I have the Corsair M60 but I think I will be switching to the Logitech G500s soon.
Logitech G500s, evenly placed weights and a classic Logitech design make it my mouse for the next few years.
I was using a Steelseries Sensei Pro, very comfortable but software not great. It bombed on me after 9 months so I went back to my Razer Mamba. Which, although not great, the side rubbers started coming off after 6 months, still works. I may look at a simple no frills (in terms of superfluous buttons) Steelseries Kinzu Pro optical mouse next.
Corsair Vengeance M65, the firmware patch fixes basically the only problem with it, and it's highly accurate, incredibly comfy, simplistic where it needs to be with feathery light clicks and a nice heavy construction.
Lovely rubberised grippy plastic on top and dat aluminium chassis with a brushed base.
Great mouse. Recommended.
It also feels like I could kill a man using it as a flail.
Conversely I can't get comfy with the R.A.T. series mice, and the Razer Deathadder feels like something I can crush in my hand by accident, also feels cheap.
I am not the only one!
I use a thermaltake Theron for gaming, the side buttons sadly no longer function

Just bought Corsair M65 but didn't like the feel after a while, so bought a Func MS-3. Now I just need to hope that I can solve the windows 8 issues
Me too! Been using it for over 3 years now; criminally under-rated mouse! Especially for the price tag!
I am thinking of upgrading though but purely because I've become too accustomed to 3600dpi and it just doesn't feel so responsive enough. I got this as part of my first "cheap" build after I left home and was getting back into PC gaming. Didn't want to spend too much on peripherals as I wanted more money on "important" components and this had as many buttons as I wanted and, more importantly, where I wanted. I've been so impressed though that I'm sticking with Sharkoon and will be getting the darkglider which is more or less identical, just higher dpi
Rat 7, the pinky rest is brilliant The most comfortable one I've ever had was a Roccat Kone, but it fell to the scrolling wheel fault. Tempted to pick up an XTD to see if they've fixed it, but for now I'm still happy with the Rat
Stealseries sensi, although it took me a long time to get used to the shape, and I'm still not a lover of it after all this time.
I miss my Diamondback Since Razer started storing profiles in the cloud they have gone down in my personal ratings.
Corsair M65 Solid mouse had it for about a 10 months now and still think its great
I use Logitech's M500 laser mouse. It's not particularly sexy but it works.

Still using my (second, warranty replaced) Logitech MX1000. Awesome mouse, my first proper gaming peripheral and still stands up now. The PSU for the charger was made of cheese and blew up, but an Ebay replacement has stood up well. No issues with wireless lag etc - played some mean CS without issue.
