Disk fragmentation maybe? How big is the buffer set at in winamp and what priority is the decoder running at. What timer resolution is it set at too (presuming you're on WA5.)

It was set to idle.

Hmmm, maybe not it usually is I think. Maybe you've installed some stuff that sits in memory and takes up CPU now and then?
Still, as long as it works now...

My guess would have been a USB device of some sort. Mate of has the same problem when he's using a USB ADSL modem...
