It's hard to put down just 5... But theses are definately the most memorable for me, mostly because I spent a stupid amount of time playing these during my school years.

In no specific order.
Super Mario All Stars - (a cheat I know as this is actually compilation but) I must have spent hundreds of hours playing this cartridge since we first got a SNES back in the early nineties. It exemplifies all that is great about simple mechanics, gorgeous design and great music.
Grand Theft Auto III - A hugely influential game. Few other games have had the impact of GTAIII. Again, I must have played it for hundreds of hours. I may well have played Vice City or San Andreas more but GTAIII was the first and therefore the best in the trilogy.
Zelda: A Link to the Past - I would rank this higher than Ocarina of Time because it was the first Zelda game I remember playing. It's one of those things that because of my age at the time, will always have a special place in my heart (as sappy as that sounds).
Portal 2 - For me this game is an all time great. When I play it, I always think to myself that it's so expertly crafted, timeless and beautiful that it deserves to be spoken of in the same breath as any Mario or Zelda game.
I was tempted to cheat again and say The Orange Box but as amazing as it is (one of the best deals in gaming ever!) I've decided to go with...
Grand Theft Auto: San Andreas - I know, it's the second GTA game on my list but San Andreas did something that Vice City didn't. As amazing as Vice City was and still is, it didn't capture the same awe that I felt the first time I played GTAIII. It grew over time, the more I learnt about Vice City, the more I liked it and the better it became but San Andreas was instantly incredible. Like GTAIII and all the other games on my list, it took my breath away from the first moment in a way that only true greats do. It was something that is really rare these days, a game that's bigger than it needs to be. A game that you'd still play at length a decade later. As much as I enjoy the Mass Effect, Call Of Duty*, Forza Motorsport series' of games. I doubt I'll be playing them in ten years time.
*CoD4 being a possible exception.
1.) GRAND THEFT AUTO: SAN ANDREAS - It's just stupid how this game is so amazing. I knew the map of San Andreas better than my own city.
2.) Starcraft - being korean and good at this game it is no doubt that I was a celebrity at school just for playing starcraft. I played this lots of times and made a lot of friends just because of this game.
3.) CS 1.6 - Very addicting for some reason. Looking back at this game, I am surprised how hard it is compared to fps games now such as battlefield 3.

5.) Need for Speed High Stakes
