When mine died on me (8months after release) Heres what happened:
Monday: Called MS, logged call, received stuff to stick on box via e-mail
Tuesday: Collected via UPS first thing in the morning
Wednesday: E-mailed to say recieved and testing
Thursday: Confirmed Broken and said will be getting a new one
Friday: Received new console (they said new, looked new, had that new smell) mid afternoon
The only bad thing was the console was without power brick (which they said i MUST send) - Called them up soon as I recieved the box, they said sorry and I got the powerbrick on saturday with a free game.
Couldnt fault it at all (apart from the console dying in the 1st place)

My 360 died a couple of months back. At the time MS said the turnaround was 5 weeks. It was particularly galling cos I was in the middle of Bioshock at the time!
Didn't actually take 5 weeks, was only about 2, got a brand new one back as well so was quite pleased. Only downside is the new one is much noisier than the old one was, but then my room isn't exactly silent anyway!
I was specifically told not to send anything other than the console itself though, no power pack or HDD or anything.

When mine died I was also sent a letter basically saying that due to long wait times they have replaced the console with a new one... took about 2 and a half weeks to get the replacement
Manufacturer date was august this year, but I really think in all honesty its a refurb with a new case/serial number - its a lot noiser than my old machine and also is a bit tempermental locking up and freezing on games at the moment...
when i sent mine back for the 4th time they actualy fixed it by replacing the whole motherboard. previosly it was a refurb that had a new heatsink. it was working amazing without no freezes or issues and then one day it packed up. i was expecting them send yet another messed up refurb but instead they fixed it! all is running fine now and hope it stays that way.
Im gunna start buying more pc games so i dont have to rely just on the 360 for gaming, starting from tomorrow its cod4 for the pc thanks you very much

From me dropping mine off at UPS to having to back took 2 weeks.
As for the new HS's I'm not sure, when it came back there was a note inside saying the motherboard had been replaced, but it never said it was a new one or not. I'd guess that if it was a new motherboard then it would have the new HS's but more than likely it will have been a refurb or older stock.

I'm bit disappointed about it taking so long to come back....after resisting buying a xbox live subscription for 10months, I get the Halo 3 Special Edition 12month+1free and my 360 dies on me nearly straight away.
