People who regularly send emails, usually with requests for work to be done, with a blank subject line - makes it so much more difficult to browse through archives
Actually, emails and posts which meaningless subject lines too - "ugh!", "WTF?", "OMGROFLMAO!!!1!1one!1!", etc. and those with half-finished questions - "Do you think...", "How about...", and so on.

Something my mother did the other day while I was sitting in the kitchen watching Top Gear. The phone rang and she answered it, so being a considerate son I muted the TV.
